Gateways to Opportunity Forms and Applications

Gateways to Opportunity offers several programs and services for you. If you are applying for the Gateways Registry only, apply online for a quick and easy membership option. Processing a paper Form takes approximately 30 days.

The first step to apply for a Gateways program is to complete the Gateways Registry Membership Form. Follow the steps below to download one PDF for the program(s) you want to apply for or renew.

Step 1—SELECT one form below.

	Gateways Registry Membership Form—Complete to become a Gateways Registy member. A Gateways Registry Membership Form must be completed to apply for other Gateways programs.
	Information Update Form—Complete this form if you already are a Gateways Registry member, are renewing your current participation, or have participated in a Gateways program before.
	La Forma de Membresía del Registro de Gateways—Complete para hacerse miembro del Registro. La Forma de Membresía del Registro de Gateways tiene que ser completada para aplicar a otros programas de Gateways.

Step 2—CHOOSE supplement applications to participate in one or more programs. (optional)

	Credentials—Offers the Illinois Director, ECE Level 1, Infant Toddler, and ECE Credentials.
	Gateways Registry Trainer/Trainings—Offers the opportunity for you to become a Gateways Registry-approved trainer and submit a training.
	Scholarship Program—Offers scholarship money to return to school.
	Great START—Offers a wage supplement for already earned education and commitment.

Step 3—DOWNLOAD.

Simply click below and print on white paper. Fill it in using blue or black ink and return with all required documentation (a checklist is provided in the application to help).
